How to submit a claim

When submitting a claim, we ask that you first complete and sign a claim form. The claim forms for the policy you hold may be accessed from the links below: Please note for all travel claims, you must also submit a copy of your travel itinerary along with your claim form.

Please note, the acceptance of your claim form does not represent an admission of policy liability on the part of BHSI. We will review and assess each claim on its’ own merits and all settlement decisions shall be determined according to the terms and conditions of your policy.

We reserve the right to ask for documents  (including originals) and information in addition to that which you submit with your claim form. The production of original documents or other statements in support of the claim shall be the responsibility of the Policyholder/Insured Person (as applicable).

Claims involving third party property damage or bodily injury
Property loss or damage
  • If you suffer a theft, robbery, the loss of money, personal effects or travel documents, the incident should be reported to the police at the location where the incident took place within 24 hours. You should request for a copy of the police report and submit this to us with your claim.
  • If your baggage, personal effects or other property are damaged whilst in the custody of an airline, rail company, cruise operator, coach/bus company or other transportation operator, you should report the matter to them immediately and make a formal request for a Property Loss/Damage Irregularity Report or similar report. The report should be submitted to us with your claim.
  • Details of the make and model of the items lost or damage along with evidence of ownership for the items being claimed in the form of proof of purchase (eg receipts, tax invoices, bank statements).
  • For damaged items, please submit a copy of the repair quote/invoice along with photos showing the extent of damage.
Vehicle related claims
  • If you are involved in a motor accident, the incident should be reported to the police at the location where the incident took place within 24 hours. You should request for a copy of the police report and submit this to us with your claim.
  • The following documentation / information should also be submitted along with your claim:
    • Your vehicle rental agreement showing the excess amount charged by the rental company
    • A copy of the repair invoice (please contact your rental company who will provide you a copy)
    • Proof of any towage charges incurred
Medical related claims
  • The following documentation / information should be submitted along with your claim:
    • Itemised medical bills and receipts
    • Medical report from the attending doctor
    • MRI/CT Scan/Histology/X Ray/Lab Reports and any letter of referral your doctor may have given you (if applicable)
    • Hospital Discharge Summary (if applicable)
  • If making a claim under Expatriate Medical Policy or Group Personal Accident Policy, please also include:
    • A copy of your passport and/or identity card
    • A copy of your pay slips for the six months preceding the accident or sickness (if a claim for weekly benefit is being made)
Accidental death claims

In the unfortunate event that the Insured Person is fatally injured following an accident, the following documentation should be submitted with the claim:

  • A copy of the death certificate
  • Documents in support of the relationship between the deceased and the party making the claim (if applicable)
  • Police Report
  • Police Investigation Report (if any)
  • Post Mortem or Toxicology Report (if any)
  • A Coroner’s Certificate with an investigation report and the coroner’s findings
  • If the death occurred overseas and involved a Singaporean Citizen or Permanent Resident, a letter confirming the death issued by the Singapore Immigration & Checkpoint Authority

Travel/baggage delay or cancellation claims

The following documentation/information should be submitted along with your claim:

  • Evidence of the delay or cancellation. This may include a letter from the airline or other transport provider confirming the delay or cancellation
  • Details of any refund received or credit given by any airline, transport operator, hotel or other third party
  • Evidence of any additional costs incurred by you due to the delay or cancellation
  • Medical reports and/or other certification evidencing the bodily injury, sickness or death that led to the cancellation and disruption to the journey (if applicable)

Claims involving third party property damage or bodily injury

Where practicable, the following documentation/information should be submitted along with your claim:

  • Particulars and contact details for the third party
  • Details of the nature and extent of the property damage (eg photos, videos)
  • Details of the bodily injury caused to a third party
  • Details of the circumstances in which the damage arose
  • Details of any witnesses to the incident including contact details
  • Details of any claim submitted to you by a third party in writing

IMPORTANT: At no time should you admit liability, settle or negotiate a claim without our prior approval. Instead the third party should be directed to contact BHSI.
